Professional Referrals

Thank you for referring your client to the Center for Holistic Sexual Wellness. We provide pleasure-based, trauma-informed sexual wellness coaching and educational support for individuals and partners.

This referral form is a tool for coordinated care when sexual concerns intersect with trauma, medical conditions, or relational complexity.

This form facilitates collaborative care. It does not replace the client’s intake process.

The Center for Holistic Sexual Wellness provides educational and coaching services. We do not provide psychotherapy or medical treatment. Clients requiring licensed mental health or medical intervention will be referred accordingly.
